CaPA 2024 Impact Report Form

CaPA has updated our Grant Report form to collect information that will allow us to both evaluate our collective impact in 2024 and lay the groundwork for our grantmaking to be streamlined in future years. If you would prefer to complete the grant report form that was originally shared in award contracts (if your contract required reporting), you are welcome to do so – however, we believe that this new version will be easier to complete and better prepare CaPA to share your accomplishments and capabilities with prospective funders.

Organizations that did not receive funding from CaPA in 2024 are still encouraged to complete this form as CaPA’s donor’s geographic priorities are regularly shifting. CaPA will also be sharing the information collected via this form with large donors and grant giving entities via a private version of CaPA Connector platform.

There are three sections on this form: 1) a request to update or add your organization’s information to the public CaPA Connector platform on CaPA’s website; 2) a budget worksheet used to inform CaPA and other institutional givers about the finer details of your work; and 3) two short answer questions about your work this past year.

If CaPA awarded funding (or if you are interested in receiving future funding) to both your nonpartisan and political affiliates programs, please complete a submission form separately for each entity type. While updating a shared c4/PAC political card is fine, our preference for 2025 is to have each entity type have its own CaPA Connector Card. Please use the submission link provided to you to complete this form multiple times for each one of your affiliate entities with your Organization Name and entity type adjusted in that first question.

To aid your work on this submission: Here is a Google document with all the questions in the 2024 Impact Report form. We recommend you make a copy of this form for group editing with your team. Your Budget Worksheet accessible from this form below in particular may have different team members working on it and is already set up for group editing. This online form displays your previous responses (if you already have a CaPA Connector card) which you can print and save as PDF to have an offline copy of those responses.

UPDATE YOUR PUBLIC CAPA CONNECTOR CARD

To help raise money for community-based organizations that are engaging voters, CaPA has created the CaPA Connector platform to increase the visibility of your great work among donors both inside and outside our network.

Please review this webpage – https://cleanprosperousamerica.org/organizations/ and find your organization’s “display card” which are in alphabetical order. Control/Command + F is an easy way to search the full list for your org name.

If there is anything in the listing you would like to change, or if your organization does not yet have a card, please use the fields below to update your information. Only fill in form fields you want to change or share, leave blank any fields that do not need to be updated.

Please enable JavaScript in your browser to complete this form.

Unique ID format established by full legal name (entity type).

Note: This Unique ID is assigned based on your previous submission. Many entities submitted information for both their c4 & PAC entities combined, in which case the Unique ID represents that dual entity representation. In 2025 we are looking to have each entity submit its own card so please  submit this form for each entity type you would like listed in the CaPA Connector.

Is your organization name correct above? If yes, leave this field blank. If not, or if you are separating out a previously joint org info card please fill in your Organization’s Name here followed by the entity type (c3, c4, or PAC) in parenthesis.

Do not display the org
Entity Type

Check all that apply for this card. A single card should only cover either your nonpartisan organizations or your political affiliates work. While c4/PAC political programs have previously been displayed on a single card, we are encouraging orgs to submit a new card for every individual entity they would like highlighted in the CaPA Connector even if they have nearly identical programming plans. You can start a new org card as needed by using the submission form link emailed to you. Please use this same link and form multiple times to submit the appropriate information for each one of your affiliates. CaPA will remove any shared org cards already on file when individual org card(s) are submitted for those affiliates.

Previous Response: {acf_program_states_acronyms}

Below are the sub-state geographies defined by congressional district boundaries that your efforts engaged in 2024, as listed on CaPA’s website. Does this list capture all sub-state geographies where your organization is currently engaged? Please add any congressional district geographies where you fielded work in 2024 utilizing this format (State Acronym - District # separated by commas) [e.g. CA-27, AZ-01, AZ-06, CO-03] Please note this does not mean or require that your organization engaged the congressional district race, just the geography contained by those district boundaries. Many of our donors are geographically specific where they want to route resources so please make sure this list is as comprehensive as possible and irrespective of the competitiveness of the congressional district.

2024 VOTER ENGAGEMENT PROGRAM BUDGET WORKSHEET

Please complete this 2024 budget worksheet to the best of your ability to represent your actual (estimates are fine) 2024 direct program budgets, volunteer recruitment, content creation, and overhead expenses. Please enter your budget worksheet data directly in the Google worksheet link provided. Do NOT make a copy, download or otherwise modify the provided link where your information should be entered.

While this worksheet is only required for organizations that had a reporting requirement in their CaPA award contract, we encourage all groups to complete it since we will share this information with interested institutional donors and we have found this data greatly strengthens your overall profile on our private CaPA Connector+ platform. The budget worksheet information will not be displayed on the public CaPA Connector website.

If your organization already submitted a budget worksheet you can reference it below to aid the completion of your new 2024 estimate of actual budget worksheet. If we do not currently have a budget worksheet for your organization, that field will be empty on the form. We have found many of our institutional donors use budget worksheets as their primary tool for evaluating programs they are considering funding so please take your time to get these numbers as granular and as accurate as possible. Please watch this short video (coming soon), or read the information notes in "column I” before completing your budget worksheet to ensure general consistency among organizations’ submitted data.

Please note we are NOT INTERESTED in trying to compare your original submission to actual budget deployment. Please do not feel any pressure to align with your original submission. We are VERY INTERESTED in accurately reflecting the scale and the associated costs associated with running any programs you fielded in 2024.

FINAL GRANT REPORT QUESTIONS

Please share reflections, observations or trends from your experience doing outreach and engaging with your communities in 2024 that you think donors and partner organizations should be aware of. This could include – but is not limited to – which voter engagement tactics were more (or less) effective; what messaging resonated (or fell flat) with voters; or notable comments and patterns that you saw/heard in the field that may explain the shift in voter behaviors in 2024.

How much of the funding that you received from CaPA was spent engaging 18-35 year olds in non-urban areas (rural areas and small cities that are not part of suburbs of cities with >100k people)?  We are collecting this information to assess which organizations’ programs may have contributed to the mission of the Rural Youth Voter Fund, a partnership between CaPA, Movement Voter Project (MVP) and the Rural Democracy Initiative (RDI). If you received funding from MVP or RDI for this rural youth focused work, please do not double report the attribution here; just say zero dollars, or only report the dollars that do not overlap with funding from RDI and/or MVP.
