CaPA 2025 Impact Report Form

All organizations that received funding from CaPA in 2025 are encouraged to complete this report. If your organization received $5,000 or more, this report is required unless your grant contract has no reporting requirements listed. We encourage you to complete the report regardless, as we will share the information collected with donors and grantmaking organizations, which may lead to future funding opportunities. 

This report has three section:

  1. Information about your organization that will be used to update your profile on the public CaPA Connector platform on CaPA’s website and shared with our network of trusted donors on a private version of the platform.
  2. A budget worksheet used to inform CaPA and other institutional givers about the finer details of your work this past year.
  3. Three short-answer questions about your work this past year.

If CaPA awarded funding to more than one of your organization’s affiliate entities (501c3, 501c4, PAC, etc.) please complete a separate report form for each entity. However, you can complete one single Budget Worksheet for all your entities combined. For any of your entities that did not receive funding, you are welcome to fill out the CaPA Connector Profile Creation Form to be added to the CaPA Connector.

To aid your work on this submission: Here is a Google document with all the questions in the 2025 Impact Report form. We recommend you make a copy of this form for group editing with your team.

To help raise money for community-based organizations that are engaging voters and advocating for climate, social, racial and/or economic justice, CaPA has created the CaPA Connector platform to increase the visibility of your great work among donors both inside and outside our network.

Please review this webpage – https://cleanprosperousamerica.org/organizations/ and find your organization’s “display card” which are in alphabetical order. Control/Command + F is an easy way to search the full list for your org name.

Some entities submitted information for both their c4 & PAC entities combined, in which case the Unique ID reflects that dual entity representation. 2025 PROGRAM BUDGET WORKSHEET

Please complete this budget worksheet to the best of your ability to represent your actual (estimates are fine) 2025 direct program costs and overhead expenses. Please enter your budget worksheet data directly in the Google worksheet link provided. Do NOT make a copy, download or otherwise modify the provided link where your information should be entered.

While this worksheet is only required for organizations that had a reporting requirement in their CaPA award contract, we encourage all groups to complete it since we will share this information with interested institutional donors and we have found many of our donors use budget worksheets as their primary tool for evaluating programs they are considering funding. The budget worksheet information will not be displayed on the public CaPA Connector website.

FINAL GRANT REPORT QUESTIONS

Please share reflections, observations or trends from your experience doing outreach and engaging with your communities in 2025 that you think donors and partner organizations should be aware of. This could include – but is not limited to – which engagement tactics were more (or less) effective; what messaging resonated (or fell flat) with voters; or notable comments and patterns that you saw/heard in the field.

Briefly describe how support from CaPA has helped your organization prepare for your programs in the coming year(s). If applicable, we are especially interested to hear how your work in 2025 has laid the groundwork for any electoral programs you plan to run in 2026.

How much of the funding that you received from CaPA was spent engaging 18-35 year olds in non-urban areas (rural areas and small cities that are not part of suburbs of cities with >100k people)? Please provide a dollar amount. We are collecting this information to assess which organizations’ programs may have contributed to the mission of the Rural Youth Voter Fund, a partnership between CaPA, Movement Voter Project (MVP) and the Rural Democracy Initiative (RDI). If you received funding from MVP or RDI for this rural youth focused work, please do not double report the attribution here; just say zero dollars, or only report the dollars that do not overlap with funding from RDI and/or MVP.
