To help raise money for community-based organizations that are engaging voters and advocating for climate, social, racial and/or economic justice, CaPA has created the CaPA Connector platform to increase the visibility of your great work among donors both inside and outside our network. CaPA also uses the information collected via this form to inform our grant program.

If your organization does not yet have a card, please use the fields below to share your information. You are welcome to leave fields blank if there is any information you’d prefer not to share, though we strongly recommend providing as much information as possible so your profile is complete. At the end of the form you will be asked to attach a Statement of Financial Activity (Profit & Loss) for the last full fiscal year. If you do not have this available, submit the form and it can be added later.

Each legal entity (c3, c4, PAC, etc.) should have a separate card. Please complete this form separately for any affiliated entities that you would also like to be considered for funding. Note that organizations listed on the CaPA Connector should be based in and primarily working in the United States.

Please provide the last 4 digits of your entity's bank account for receiving ACH and wire transfers. This information is collected to avoid the need to verbally verify your account information if you receive a grant award from CaPA and its affiliates.
If applicable, provide the name of your organization’s fiscal sponsor. If you do not have a fiscal sponsor leave this field blank.

Check which other entity types, if any, are affiliated with your organization. Please list the full legal name and Employer Identification Number (EIN) for each other entity type that is seeking funding.

Please share with us all of the Congressional Districts where your organization engages with communities directly (e.g. include districts where you conduct direct/in-person outreach but not every district your social media/digital ads reach). Many donors are interested in work that is occurring in specific areas and CaPA tracks sub-state geographies using U.S. congressional districts. This does not mean or require that your organization engages the congressional district race(s), just the geography contained by those district boundaries. Please attach a Statement of Financial Activity (Profit & Loss) for the last full fiscal year. If your organization does not maintain accounting records of expenditures, provide a brief summary of your annual expenses.

While this document is not required, we encourage all groups to provide it since we will share this information with interested donors and we have found many of our donors use financial information as a tool for evaluating programs they are considering funding. The document you provide will not be displayed on the public CaPA Connector website and will only be shared with trusted funding partners.
