DO NOT FILL OUT THIS FORM IF YOU HAVE ALREADY APPLIED TO CaPA FOR FUNDING UNDER OUR OPEN RFP’s. THIS FORM IS JUST FOR GROUPS THAT WANT TO PROVIDE THE MINIMUM INFORMATION REQUIRED TO BE DISPLAYED PUBLICLY ON THE CaPA CONNECTOR WEBSITE – BUT ARE NOT APPLYING FOR CaPA FUNDING.

This is not a Request for Proposals and will not get you considered for CaPA derived funding. If you would like to both apply for funding, and get listed on our CaPA Connector public website, please respond to our Open Requests for Proposals which have more detailed intake forms.  If you are not concerned about applying for CaPA derived funding but just want to quickly get your organization listed on the CaPA Connector website, please complete this form which gathers the minimum required information to generate a public organization display card on our website.

Please indicate your organization's legal name(s). 501(c)3 entities should apply as individual entities. Political affiliates like 501(c4)’s and PAC’s will be displayed on a single org display card and should provide a joint submission with both legal entities names and entity type listed in response to this question. If you have both partisan and c3 entities that you would like displayed please complete this Public Card Info submission form twice; once for your c3 and once for your partisan affiliates.
